== Historical understanding == Historically, there has been much dispute over whether such a thing as a vacuum can exist. Ancient Greek philosophers debated the existence of a vacuum, or void, in the context of atomism, which posited void and atom as the fundamental explanatory elements of physics. Lucretius argued for the existence of vacuum in the first century BC and Hero of Alexandria tried unsuccessfully to create an artificial vacuum in the first century AD. Following Plato, however, even the abstract concept of a featureless void faced considerable skepticism: it could not be apprehended by the senses, it could not, itself, provide additional explanatory power beyond the physical volume with which it was commensurate and, by definition, it was quite literally nothing at all, which cannot rightly be said to exist. Aristotle believed that no void could occur naturally, because the denser surrounding material continuum would immediately fill any incipient rarity that might give rise to a void. In his Physics, book IV, Aristotle offered numerous arguments against the void: for example, that motion through a medium which offered no impediment could continue ad infinitum, there being no reason that something would come to rest anywhere in particular. In the medieval Muslim world, the physicist and Islamic scholar Al-Farabi wrote a treatise rejecting the existence of the vacuum in the 10th century. He concluded that air's volume can expand to fill available space, and therefore the concept of a perfect vacuum was incoherent.

It is now generally acknowledged that by itself, the low protein theory does not adequately account for the pathogenesis of kwashiorkor. More complex deficiencies are at work. These have still not been established. Social factors are also relevant and ignorance of nutrition can be a cause. A case was described where parents who fed their child cassava failed to recognize malnutrition because of the edema caused by the syndrome and thought the child was well-nourished despite the development of kwashiorkor.

== History == The US Food and Drug Administration (FDA) approved ritlecitinib based on evidence from a clinical trial of 718 participants with severe alopecia areata. The efficacy and safety of ritlecitinib were evaluated in a randomized, double-blind, placebo controlled trial in 718 participants twelve years of age and older with alopecia areata with ≥50% scalp hair loss, including alopecia totalis and alopecia universalis. The trial randomized 130 participants to ritlecitinib 50 mg once daily, 131 participants to placebo, and 457 participants to other ritlecitinib dosing regimens. The safety evaluation was also supported by two placebo-controlled trials in which 80 participants were randomized to ritlecitinib 200 mg once daily for four weeks followed by 50 mg once daily and 82 participants were randomized to placebo. The trial was conducted at 128 sites in 18 countries in Argentina, Australia, Canada, Chile, China, Colombia, Czech Republic, Germany, Hungary, Japan, Republic of Korea, Mexico, Poland, Russian Federation, Spain, Taiwan, the United Kingdom, and the United States.

The term "Arch" is derived from Greek ἀρχή (archē, 'authority') that is to say it means "chief". 'Purple' comes from one of the colours, mentioned in the Bible, which were used to make the curtains of the tabernacle (the others being Blue/Indigo and Scarlet). Some claim it is of Masonic origin; The "Diamond Boys" from North Armagh - Sloan, Winter and Wilson from the neighbouring Dyan in County Tyrone were all Freemasons. Wilson had asked his own Masonic lodge to give active help in repelling the Defenders (Ireland) and they refused, almost certainly saying that that would be against Masonic principles. As a result of that refusal and left without any alternative after the Battle of the Diamond the four men formed the Orange Order. They had no other template on which to base the proposed ritual and procedure but that of Freemasonry. That is why Orange ritual and most particularly Arch Purple ritual somewhat resembles that of Freemasonry. In fact it is so close that the Arch Purple can be considered to be a Protestant form of Freemasonry. although this has been repeatedly disputed, with overwhelming evidence proving the degree to be descended from the Orange Boys of the Dyan (prevalent around the early 1790s), which in turn based their degrees on the early Boyne Societies which dated from the late 17th century. The Royal Arch Purple Degree itself is constructed along Christian lines, with Christian faith, hope and charity being commended to the new brother.

== Interactions == Serotonin syndrome may result from the combined use of dextromethorphan and serotonergic antidepressants such as selective serotonin reuptake inhibitors (SSRIs) or monoamine oxidase inhibitors (MAOIs). The doses of dextromethorphan beyond those normally used therapeutically that can produce this effect are unknown. In any case, dextromethorphan should not be taken with MAOIs due to the possibility of this complication. Serotonin syndrome is a potentially life-threatening condition that can occur rapidly, due to a buildup of an excessive amount of serotonin in the body. Combining alcohol with dextromethorphan significantly increases the risk of overdose, according to the NIAAA. Compounds in grapefruit affect a number of drugs, including dextromethorphan, through the inhibition of the cytochrome P450 system in the liver, and can lead to excessive accumulation of the drug which both increases and prolongs effects. Grapefruit and grapefruit juices (especially white grapefruit juice, but also including other citrus fruits such as bergamot and lime, as well as a number of noncitrus fruits) generally are recommended to be avoided while using dextromethorphan and numerous other medications.
